Requesting Transcripts and Recommendation Letters

Requesting an Official Transcript
 
You may access the Transcript Request Form (pdf) from this site. Please print out the request and fill it out completely, including signatures. You may also pick up a copy from Mrs. Thomas in Guidance. You must give two weeks for transcripts to be processed.

Each form must be filled out with the following:
• Student name
• Student number
• Student signature
• Name of college or scholarship
• Address of college or scholarship
• Parent signature
• $3.00 fee for each transcript sent or picked up

Please Note: SAT scores will NOT be sent with your transcript, most colleges require official scores from College Board.
 
Requesting a Recommendation Letter
 
Letters of recommendation should be requested at least two weeks in advance from the teacher or counselor from whom you want a recommendation. Provide each recommender with a stamped envelope addressed to the college or scholarship organization. If the letter is from your counselor, a stamped envelope is not required. You should also provide your academic résumé (doc) and inform the recommender of any special circumstances that may be noteworthy.
